Well Destruction Results Report �� vmww <br /> Moore Petroleum Bulk Plant,5491 `F'Street,Banta,California <br /> Page 6 <br /> (TPHg), as diesel (TPHd), as kerosene(TPHk), and as motor oil (TPHmo). The reduced <br /> analytical suite was approved in a letter from the SJCEHD, dated July 2, 2009. <br /> June 12, 2009—Apex submitted a Monitoring Well Installation Workplan, proposing to install a <br /> deep screened well (MW-18) to a depth of 80 feet bgs via conductor casing to 52 feet bgs to <br /> determine the vertical extent of contamination at the site. The workplan was approved by the <br /> SJCEHD in a letter dated July 2, 2009, under the condition that the well be installed with 10 feet <br /> of screen. <br /> July 2, 2009—Apex received a letter from the SJCEHD, reducing the required sampling <br /> frequency for wells at the site. Under the new sampling program, wells MW-3, MW-4A, MW-7, <br /> MW-8, MW-10, MW-11, MW-12, RW-2 and VEW-2 will be sampled semi-annually and wells <br /> MW-2, MW-5, MW-6, MW-9, MW-13, MW-14, MW-16 and MW-17 will be sampled annually. <br /> The letter did not provide sampling frequencies for wells MW-1 and MW-15. The letter <br /> recommended that the SVE system be turned off to perform rebound testing. Additionally, the <br /> letter approved a confirmation soil vapor survey during the SVE system rebound testing to <br /> further assess the risk posed by vapor intrusion at the site. <br /> September 8 and 10, 2009—Apex supervised the installation of monitoring well MW-18 at the <br /> site. Drilling activities are detailed in Apex's Third Quarter 2009 Groundwater Monitoring and <br /> Monitoring Well Installation Results Report, dated November 11, 2009. <br /> 1.1 <br /> November 12, 2009—Apex submitted a Confirmation Soil Vapor Survey Workplan to the <br /> SJCEHD. <br /> V <br /> August 5, 2010—The SJCEHD issued a letter approving the November 12, 2009, Confirmation <br /> Soil Vapor Survey Workplan, with the condition that vapor probes SVP-14 and SVP-15 be <br /> analyzed for TPHd. The letter also stated that if the results of the soil vapor survey are favorable, <br /> a closure report should be prepared for the site. <br /> 1� <br /> December 10, 2010—Apex submitted a Drilling Addendum Letter to the SJCEHD. <br /> August 3, 2011 —Apex submitted a First Quarter 2011 Groundwater Monitoring Report, Soil <br /> Vapor Survey Results Report, Human Health Risk Assessment, and Closure Report to the <br /> SJCEHD. <br /> V <br /> April 19, 2012—The SJCEHD issued a letter concurring with the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB)recommendation for case closure. In the letter the <br /> SJCEHD has begun the 30-day closure notification period and has requested a workplan be <br /> submitted for proper well destruction which is included below. <br /> August 20, 2012—The SJCEHD issued a letter concerning the comments received during the <br /> 30-day public participation period for closure. Based on the concerns of the local landowners <br /> regarding health, SJCEHD requested that domestic wells on adjacent properties not previously <br /> sampled be sampled before site closure is granted. <br />
