Preparing any accident/incident reports (see attached Accident Report Form). <br /> r <br /> 1.4 Project Employee <br /> Project Employees involved in on-site investigations are responsible for: <br /> 1. Taking all reasonable precautions to prevent injury to themselves and to their <br /> fellow employees. <br /> 2. Implementing Health and Safety Program and reporting to the PSO for any <br /> action deviating from the anticipated conditions described in the Health and <br /> Safety Plan. <br /> 1.5 Special Instructions <br /> r It is the responsibility of the Project Manager and the Health and Safety Manager to <br /> review the available site information to determine, to what degree the Health and <br /> Safety Plan will be implemented. Each site will be handled on a case by case basis. <br /> This site, because of the nature and known levels of contaminates, has been identified <br /> as a Level "D" entry. Based on U.S. EPA rating criteria for entry, this site requiresa <br /> minimum of protection. This will include rubber boots, tyvek, gloves, and hard hats. <br /> Should for any reason, conditions change, the site will be elevated to an appropriate <br /> level of entry for safe working, based on the Project Safety Officer (PSO) and Project <br />�- Manager (PM). <br /> L� <br /> Should at any time the level of entry change, the remaining specific safety equipment <br /> and precautions can be implemented by the manager as required.
