10.0 Medical Surveillance Requirements <br /> 10.1 Routine Medica!Monitoring <br /> All field personnel, including subcontractor field personnel, assigned to the project shall <br /> participate in a medical surveillance program as required by 8 CCR 5192 (f), and <br /> 29 CFR 1926.65. All field personnel shall have completed an initial or updated medical <br /> 6W examination within the last twelve months (unless the examining physician has specified an <br /> alternate frequency for update examinations for an individual employee). All field personnel <br /> shall be approved to wear air purifying respiratory protective equipment as part of the medical <br /> evaluation(8 CCR 5144). Medical Surveillance for each contractor/subcontractor will be <br /> overseen by a licensed physician certified in Occupational Medicine by the American Board of <br /> Preventive Medicine or is Board eligible. <br /> �... The purpose of the medical surveillance program is: <br /> To establish a baseline of an individual's health against which, future changes can <br /> '-- be measured <br /> • To identify any underlying illness or condition that might be aggravated by <br /> chemical exposures or job activities, such as the use of respiratory equipment <br /> • To allow recognition of any abnormalities at the earliest opportunity, so that <br /> 4, corrective measures can be implemented <br /> IT shall maintain current records of participation in a medical surveillance program for each field <br /> '-' person assigned to the project. This record shall include a copy of the physician's opinions, <br /> findings, and recommendations (including approval to wear respiratory protective equipment) for <br /> ... the most recent medical examination. A copy of these records shall be maintained on-site during <br /> the project. Subcontractor personnel shall be responsible for administering a medical <br /> surveillance program for their employees, and to provide appropriate records of participation for <br /> subcontractor personnel assigned to the project. <br /> 10.2 Medica!Records <br /> Medical records associated with monitoring requirement will be maintained, as required.
