11.0 Site Documentation Procedures <br /> The implementation of the site HASP must be documented to assure employee participation and <br /> protection. In addition, the regulatory requirements must be met for record keeping on training, <br /> medical surveillance, injuries and illnesses, exposure monitoring, health risk information, and <br /> respirator ft-tests. Documentation of each employee's activities is maintained by the Regional <br /> Health and Safety Officer. In addition, documentation of the implementation of this plan and of <br /> the site safety briefing is required for this project. Appendix B contains the form to acknowledge <br /> to the reading this plan. <br /> 19.9 First Aid/Accident Reporting <br /> All injuries and accidents will be reported promptly the SSHO. Reportable incidents include: <br /> • Injuries to personnel resulting in lost time <br /> • Tool or equipment failure that results or could result in serious injury <br /> • Fire or explosion of any magnitude <br /> • Exposure of unprotected personnel to toxic agents <br /> • Vehicle accidents <br /> • Any damage to client or private property. <br /> +r <br /> All injuries/illnesses, no matter how minor they appear, are to be reported to the PM or SSHO. <br /> They should then ensure that the incident is logged and properly reported <br /> (i.e., Cal/OSHA 200 log, if appropriate). <br /> Under no circumstances should an injured employee drive himself/herself to the hospital or <br /> clinic. An employee with minor injury may be transported by car after first aid treatment is <br /> given. The employee who transports the injured person should be trained in first aid and CPR, <br /> whenever possible. In the event the injury is severe, or when in doubt of the severity of the <br /> injury, employee should be transported by ambulance. <br /> Injured employees that require medical treatment or have been taken to a doctor,hospital, or <br /> clinic should not be allowed to resume work without a written return to work statement from the <br /> treating physician. This statement should give the diagnosis, date of return to work, and any <br /> work limitations. Should the statement such as"light duty"be given, call the treating physician <br /> to determine the exact restriction that is needed.
