Maintenance of an operating log at the last location to contain the following <br />information: <br />♦ the name and address of each generator location contributing material to <br />each shipment received; <br />♦ the quantity and type of material contribute by each generator to each <br />shipment; <br />♦ The date of each shipment received or sent offsite; and <br />♦ The destination and intended disposition of all material shipped offsite <br />or received. <br />• The log must be kept for at least three years after receipt of the material at <br />that location. The shipping paper template used for logging routinely <br />generated wastes may be used for this purpose. <br />Record- AP If the facility is recycling its antifreeze on-site, an operating log should be <br />keeping: kept. This log should include the accumulation start date, the recycling date, <br />quantity, and recycling vendor information. <br />9 H&SC 25143.10 <br />9 H&SC 25143.1o(t) <br />If more than 23 gallons of antifreeze is recycled on-site in a month, then <br />'additional information must be submitted to the authorized local ' <br />enforcement agency no later than July 1 of every even numbered years. <br />However, the local enforcement agency may exempt antifreeze recyclers <br />from this requirement 9 The submission format is determined by the local <br />enforcement agency, but at a minimum, the following information must be <br />included: <br />The name, site and mailing addresses, and telephone number of the <br />facility that recycles the material. <br />The name and address of the generator of the recyclable material. <br />Documentation that the provisions of the recycling exemption are <br />met. <br />Contact the local enforcement agency for their specific guidance and <br />requirements. <br />IV -5 (t)PG&E July 1997 <br />
