Before returning any equipment to service following lockout and tagout, the following <br /> procedures are required <br />' Ensure that all <br /> • nonessential sterns (e g , tools and cleaning rags) are removed <br /> from the equipment <br /> • Ensure that equipment components are intact <br />' • Check work area to ensure that all employees are safely positioned or removed <br /> from the area <br />' • Notify all affected employees and site supervisor before re-energizing the <br /> equipment <br />' • Remove lockout/tagout device <br />' • Re-energize equipment or open valves and restore flow in process luxe, place <br /> back into service. <br /> tWhere equipment must be locked out for longer than one work shift, the individual lock(s) of <br /> the outgoing shift working on equipment will be removed and replaced by the on-coming <br /> shift's individual lock(s) The authorized employees of the on-coming shift must inspect and <br /> "try" the system to ensure de-energization The site supervisor shall re-audit the system as <br /> tnecessary <br />' 4.2.6 Use of Tools <br /> The following safe work practices apply to the use of hand tools and sampling equipment <br />' • Only use a tool for its designed use. <br /> • Do not use damaged tools <br />' • Diving faces of hammers, chisels, dnftP ins, bars, and similar tools must be <br /> inspected to eliminate mushroomed heads, broken faces and other defects <br />' • Tools must be returned to theirProper storage Place <br /> g <br />' • Sharp tools must not be tamed in pockets. <br /> • Wood handles must be sound and securely wedged or fastened to the tool Tape <br /> must not be used to cover defects such as cracks <br />' MZ/11-29-94/RHS/HASP/94-0037 JAF 4-8 <br /> 1 <br />
