Examinations <br /> All on-site project personnel associated with field activities on this project shall within the <br /> 1 past 12 months have completed a comprehensive medical examination that meets the <br /> requirements of OSHA's Regulation 29 CFR 1910 120 The annual medical includes the <br />' following elements <br /> 1 - Medical and occupational history questionnaire <br /> Physical examination <br /> - Complete blood count, with differential <br /> - Liver enzyme profile <br /> Chest X-ray, once every three years, for non-asbestos workers <br /> 1 - Pulmonary function test <br /> - Audiogram <br /> - Electrocardiogram (EKG) for persons older than 35 years of age, or if indicated <br /> ' during the physical examination <br /> - Drug screening <br /> ' W Visual acuity <br /> - Follow-up examinations, at the discretion of the examining physician or the <br /> corporate medical director <br /> ' All employee medical records are maintained by the local health and safety group within the <br /> worker's home profit center, or for subcontractors at the subcontractor's office The <br /> 1 examining physician provides the employee with a letter summarizing his findings and <br /> recommendations Each employee also has the right to inspect and copy medical records <br /> The examming physician provides the employer with a letter confirming the worker's fitness <br /> for work and ability to wear a respirator A copy of this letter for all subcontractor's <br /> MZ/11-29-94/RHS/HASP194-W37 7AP 10-1 <br />
