Background Information: Swift Roofing <br /> Page 3 of 3 <br /> On 30 April 2009,the San Joaquin County Environmental Health Department(EHD) submitted a <br /> Case Closure Summary for the site to the California Central Valley Regional Water Quality Control <br /> Board (CVRWQCB); on 08 May 2009, CVRWQCB issued a No Further Action Required <br /> Concurrence letter. <br /> On 12 May 2009,EHD issued a directive letter to destroy monitoring wells MW-1 through MW-3 <br /> under permit, in order to fulfill requirements for site closure. <br /> On 04 June 2009,groundwater monitoring wells MW-1,MW-2 and MW-3 were destroyed by over- <br /> drilling each well to its total depth with the 8.25-inch diameter hollow stem augers and a solid drive <br /> lead-bit.Wells were destroyed in accordance with the San Joaquin County Well Standards document <br /> (San Joaquin County Ordinance Code Section 9-1115.6, amended 08 May 1993), Standards For <br /> Well Construction And Destruction In San Joaquin County, dated February,2005. Well casings, <br /> screens and other materials associated with the wells were removed from the boreholes, and then <br /> each boring was grouted to near surface using a portland cement grout mixture, under EHD <br /> oversight. A ratio of six gallons water for every 94-pound sack of portland cement was utilized for <br /> preparation of the grout mixture.Auger returns generated by drilling were containerized in properly <br /> labeled Department of Transportation(DOT)-approved model 17H 55-gallon drums and were stored <br /> on-site. <br /> i <br /> Advanced GeoEnvironmental,Inc. <br />
