Request for payment form (appendix C)- submitted to the UST cleanup fund <br /> section chief with appropriate invoices the time of the payment requests. <br /> The EAR account is to be used primarily to: <br /> Provide funding to LIAs to initiate corrective action at petroleum sites where the <br /> responsible party is either unable or unwilling to take the required corrective <br /> action in response to a LIA corrective action order issued pursuant to Section <br /> 25299.37 ( c ) of Chapter 6.75 (recalcitrant sites) <br /> Agencies requesting funds from the EA Account shall provide the following site <br /> specific information to the DCWP chief: <br /> • name and address of LIA requesting funds <br /> • name and address of all identified potentially responsible parties <br /> • location of the unauthorized release <br /> • description of same <br /> • description of petroleum products released <br /> • description of water body threatened <br /> • description of threat to health, safety and environment <br /> • copy of the cleanup and abatement order or other corrective action order <br /> issued by the LIA <br /> • description of why the responsible party cannot or will not cleanup the <br /> contamination or abate its effects <br /> • documentation of any disputes between the regulator agency and the <br /> responsible party <br /> • any actions which have been previously taken to cleanup or abate the effects <br /> of the unauthorized release <br /> • description of how the proposed cleanup or abatement s to be accomplished <br /> • annual cost estimate and total cost estimate for the proposed cleanup and <br /> abatement including staff costs <br /> • description of the results if the funding should be denied <br /> Non-Emergency Funded Sites <br /> Complete the boiler plate contract LN& IN-PACKETJ <br /> A description of the proposed scope of work, an estimated budget, and proof of <br /> insurance ($1,000,000 liability insurance) is required before the contract can be <br /> initiated. Also a LIA resolution authorizing the LIA to enter into the contract is <br /> required before the contract can be approved. The LIA resolution must include: <br /> 1) wording to authorize all future amendments so that a resolution is not <br /> required for each amendment <br />
