Infurna Jr. P.G., S.R.E.H.S. <br /> February 14, 2006 <br /> Page 2 <br /> work plan with a site-specific Health and Safety Plan (HASP), a completed boring permit <br /> application, a completed Master File Record (MFR), permit and review fees, and a <br /> summary report. <br /> FIELD INVESTIGATION <br /> As required by the SJCEHD and compliant with Occupational Health and Safety <br /> Administration (OSHA) guidelines (29 CFR 1910.120) and with California Occupational <br /> Health and Safety Administration (Cal-OSHA) guidelines (CCR Title 8, Section 5192), <br /> SECOR prepared a site-specific HASP. SECOR completed the HASP with appropriate <br /> field personnel and site-specific safety information. Field staff reviewed the HASP before <br /> beginning field operations at the Site and had a copy of the HASP with them as they did the <br /> field work. <br /> Prior to the commencement of field activities, SECOR submitted to SJCEHD the required <br /> work plan, HASP, permit applications and forms, boring permit and MFR fees, and a <br /> proposed schedule. SJCEHD approved the work plan and issued boring permit SR # <br /> 0045181. A copy of the soil boring permit is included in Attachment 1. Also prior to field <br /> activities, the Site was marked for utility clearance and SECOR contacted Underground <br /> Service Alert (USA) to mark various agency sub-surface utilities (Reference Number <br /> 473230). <br /> On December 19, 2005, SECOR field staff mobilized to the Site to complete the scope of <br /> work. During advancement of the first soil boring, the impact sampler broke while staff <br /> attempted to collect the two-foot sample in the first boring. SJCEHD staff indicated that <br /> they would return the following day to observe the backfill of the hand-augered borings. On <br /> December 20, 2005, SECOR field staff remobilized to the Site and hand augered four soil <br /> borings (SB-1 through SB-4) at the locations shown on Figure 3. The locations of the <br /> borings were based on information from the PG&E yard manager (Mr. Mike Renwig) on the <br /> locations of the former con-vault and the locations of the former fill ends and fuel <br /> dispensing area. <br /> Borings SB-1 through SB-4 were hand augered to depths of approximately 3.3 to 3.5 feet <br /> below ground surface (bgs). The proposed depth of 5 feet bgs was not achieved in any of <br /> the borings due to refusal on what appeared to be a hard pan layer approximately 3.3 to 3.5 <br /> feet bgs beneath the former con-vault location. Borings SB-1A and SB-113 were step-out <br /> borings used to evaluate the refusal of the hand auger. <br /> Four discrete in-place soil samples (SB-1@2', SB-2@2', SB-3@2', and SB-4@2') were <br /> collected from the borings with a clean impact sampler at depths of two feet bgs. Deeper <br /> samples were not collected. The soil samples were collected with a decontaminated <br /> _ impact sampler in new brass sample sleeves and were field-screened for volatile organic <br /> compounds using a calibrated photo-ionization detector (PID). The samples sleeves were <br /> immediately covered with Teflon sheets and plastic end caps, labeled, recorded on a chain- <br /> I:\PG&E\report\Final Phase 11 report.doc <br />
