I - t <br /> ♦ If the driver of the vehicle is not familiar with directions to the hospital, a second person shall accompany the driver and the injured <br /> employee to the hospital <br /> ♦ If it is necessary for the SHSO to accompany the injured employee, provisions must be made to have another employee, properly <br /> trained and certified in first aid, to act as the temporary SHSO. <br /> ♦ If the injured employee is able to return to the jobsite the same day, he/she should bring with him/her a statement from the doctor <br /> containing such information as: <br /> • Date <br /> • Employee's name <br /> • Diagnosis 1 <br /> • Date he/she is able to return to work, regular or light duty <br /> • Date he/she is to return to doctor for follow-up appointment, if necessary <br /> • Signature and address of doctor <br /> If the injured employee is unable to return to the jobsite the same day, the employee who transported him should bring this information <br /> back to the jobsite and report it to Mary Harris in Human Resources at (619) 718-9429 and the Director of Industrial Hygiene and <br /> Health & Safety, Philip Platcow at (617) 232-7355. <br /> Emergency Cases Requiring Ambulance Services <br /> ♦ Medical cases requiring ambulance services would be such cases as severe head injuries, amputations, heart attacks, etc. <br /> ♦ Should ambulance service be necessary, the following procedures should be taken immediately. <br /> • <br /> Contact necessary ambulance service and company emergency services by dialing 911 or (209) 461-641 and notify the SHSO for <br /> the site. <br /> • Administer first aid until ambulance service arrives. <br /> • While the injured employee is being transported, the SHSO should contact the medical facility to be utilized. <br /> • One designated representative should accompany the injured employee to the medical facility and remain at the facility until final <br /> diagnosis and other relevant information is obtained. <br /> PG&E SECOR International Incorporated <br /> SECOR Project No.050T.50234.00.0003 PG&E French Camp <br /> HASPB.doc <br /> 4 <br />
