Copies of consolidated manifest receipts for the last three years were not found on site. There were no disposal records found for the parts washer waste. Only one consolidated manifest receipt was found for used antifreeze, dated 5/2/17. No used oil records were found for April 2015 through September 2015, and at least one manifest from October 2015 and November 2015 were not found on site. Several manifests were not found on site from 2016, including at least one in January, March, May, and June. No used oil records were found for dates between 8/18/17 through 11/22/17. Based on the disposal records found on site, used oil is picked up for disposal approximately 2-3 times a month. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all missing manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. The following consolidated manifest receipts were missing the required information: AES on 6/16/17 for used oil was missing the TSDF identification number and the waste minimization statement AES on 4/18/17 for used oil was missing the TSDF name,the TSDF address,the TSDF identification number, waste minimization statement, and the facility contact name AES on 4/4/17 for used oil was missing the TSDF identification number and the waste minimization statement AES on 1/25/17 for used oil was missing the TSDF identification number and the waste minimization statement World Oil on 3/7/17 and 2/21/17 for used oil was missing the facility contact name World Oil on 5/2/17 for used antifreeze was missing the facility contact name Safety Kleen on 1/11/16 for used oil was missing the TSDF name,the TSDF address, the TSDF identification number, and waste minimization statement All copies of each receipt shall contain all of the following information: -The name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator's representative; -The date of the shipment; -The manifest number; -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream type listed in subdivision(c), and its proper shipping description, including the hazardous class and United Nations/North America(UN/NA)identification number, if applicable; -The name, address, and identification number of the authorized facility to which the hazardous waste will be transported; -The transporter's name, address, and identification number; -The driver's signature; -A statement, signed by the generator,certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be economically practicable. Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all required information. This is a minor violation.
