N <br /> page 2 <br /> 1817 Fresno Ave <br /> In correspondence dated August 20, 1998 PHS/EHD requested that a well survey be <br /> done which documents all potential sensitive receptors located within a 2000-foot radius <br /> of the subject site. The results of the survey performed by Clearwater were included in <br /> the report, but the referenced Table 3, which details the well construction and use data, <br /> was not. Please have Clearwater forward a copy of Table 3 to PHS/EHD at their earliest <br /> convenience. <br /> On June 21 , 1999 a routine quarterly groundwater monitoring was performed on site. At <br /> this time it was discovered that MW-5 has been damaged by construction activities. The <br /> well box, concrete setting and approximately 4 feet of the well casing had been pulled <br /> out of the ground. Please have your consultant evaluate the extent of the damage done <br /> to this well , and then submit their recommendations to PHS/EHD in a written plan for <br />;I either repairing or replacing the well. Should the well require replacement, it will first <br /> have to be properly destroyed by methods outlined in the San Joaquin County Well <br /> Standards. <br /> Please prepare a work plan to continue the investigation of the vertical and lateral extent <br /> of petroleum hydrocarbon contamination at this site.
