page 2 <br /> 1817 Fresno Ave <br /> PHS/EHD requested in the August 20 , 1998 correspondence that a well survey <br /> be conducted which provides the location of all agricultural , industrial , domestic, <br /> municipal and other wells within a 2000 foot radius of the site, and that this well <br /> survey be included in the work plan . There was no well survey included in the <br /> work plan submitted to PHS/EHD . <br /> Section 2 .2 Installation of Monitoring Wells , on page 2 of the work plan , <br /> describes how the proposed monitoring wells will be constructed . It states that <br /> the sand filter pack will extend "from total depth to six-inches above the top of the <br /> screened interval . A bentonite seal one foot thick will be placed over the filter <br /> pack" . Figure 3 , Proposed Well Construction Details shows a diagram of the <br /> proposed wells as having filter pack extending one foot above the screen interval <br /> and a bentonite seal two feet thick. PHS/EHD recommends the sand filter pack <br /> be placed at least one foot above the screen interval . Please clarify to PHS/EHD <br /> the well construction detail that will be used .
