SCOPE OF WORK <br /> In accordance with PHS-EHD procedures, the on-site well will be pressured grouted to the surface <br /> with Portland cement. The top three(3)feet of the well will be overdrilled and filled with native soil <br /> to surface grade. <br /> 4.0. PROCEDURES <br /> For the bandondmnet of the on-site well, the following procedures wil be performed. <br /> 1) Prepare a Site Health and Safety Plan in accordance with the Occupational Safety and Health <br /> Guidance Manual for Hazardous Waste Site Activities - Prepared by National Institute for <br /> Occupational Safety and Health, Occupational Safety and Health Administration, U.S. Coast <br /> Guard and U.S. Environmental Protection Agency, October 1985. <br /> 2) Notify the San Joaquin County Environmental Public Health Services - Environmental Health <br /> Division (PHS/EHD) 48 hours prior to commencement of any work. <br /> 3) The well box will be removed. <br /> 4) The well will then be pressue grouted with Portland cement for approximately five minuets <br /> to ensure grout penetration. <br /> 5) The top three (3) feet of the well will be overdrilled and filled with native soil to durface <br /> grade. <br /> 6) Prepare a letter colillri ning me ,,=reii abandoIimera. <br />
