ATC Associates Inca <br /> Quality Assurance/Quality Control Plan <br /> Revised November 2002 <br /> 5.2 Chain-of-Custody Record and Sample Analysis Request Form <br /> A chain-of-custody record for each container or sample will be used to track possession <br /> of the samples from the time they were collected in the field until the time they are <br /> analyzed in the laboratory. <br /> The chain-of-custody record will contain the following information: <br /> ✓ site name <br /> ✓ signature of collector <br /> ✓ date and time of collection <br /> ✓ sample identification number(s) <br /> ✓ number of containers in,sample set <br /> ✓ description of sample and container(s) <br /> ✓ name and signature of persons, and the companies or agencies they <br /> represent, who are involved in the chain of possession <br /> ✓ inclusive dates and times of possession <br /> ✓ requested analysis for each sample <br /> 5.3 Delivery of Samples to Laboratory <br /> Samples will be delivered to the laboratory on a daily basis. Samples will be refrigerated <br /> to approximately 4°C for shipping. If a courier is used the shipping containers will be j <br /> sealed with security tape to assure sample integrity during shipping. Delivered samples E <br /> will be accompanied by a chain-of-custody record.
