4707 Pacific Ave <br /> Page 2 <br /> Unocal proposed to install one additional well to replace monitoring wells, MW1 2 and MW13; <br /> however, given the current groundwater monitoring capabilities, additional monitoring wells <br /> will be required, so that representative groundwater quality data can be obtained. <br /> Please submit a work plan and permit applications for the proposed destructions and <br /> installations. Pursuant to City of Stockton encroachment permits, two permits will be <br /> necessary for the destruction of MW10 on March Lane and of MW12 and MW13 on Pacific <br /> Ave. A destruction permit will also be necessary for the on site monitoring well, MW3. The <br /> PHS/EHD fee for a destruction permit is $60. One installation permit is required for each <br /> parcel drilled upon. The number of installation permits will be dependent on the proposed <br /> locations of the replacement wells. The PHS/EHD fee for an installation permit is $89. <br /> PHS/EHD understands that the City of Stockton has requested that field work, associated with <br /> the destruction of the four monitoring wells, be completed prior to May 17, 1996. Once the <br /> work plan and permit applications have been submitted, PHS/EHD will expedite review, so as <br /> not to delay the project.
