4707 Pacific Ave
Page 2
Unocal proposed to install one additional well to replace monitoring wells, MW1 2 and MW13;
however, given the current groundwater monitoring capabilities, additional monitoring wells
will be required, so that representative groundwater quality data can be obtained.
Please submit a work plan and permit applications for the proposed destructions and
installations. Pursuant to City of Stockton encroachment permits, two permits will be
necessary for the destruction of MW10 on March Lane and of MW12 and MW13 on Pacific
Ave. A destruction permit will also be necessary for the on site monitoring well, MW3. The
PHS/EHD fee for a destruction permit is $60. One installation permit is required for each
parcel drilled upon. The number of installation permits will be dependent on the proposed
locations of the replacement wells. The PHS/EHD fee for an installation permit is $89.
PHS/EHD understands that the City of Stockton has requested that field work, associated with
the destruction of the four monitoring wells, be completed prior to May 17, 1996. Once the
work plan and permit applications have been submitted, PHS/EHD will expedite review, so as
not to delay the project. PHS/EHD has enclosed several permit applications for your use.
If you have any questions, please contact Mary Meays at (209) 468-0337.
Donna Heran, REHS, Director
Environmental Health Division
Y V G
Mary Meays, Senior REHS Margaret Lagorio, REHS
Site Mitigation Unit Lead Senior
MM\4707PAC
cc: Elizabeth Thayer, CVRWQCB (w/o enclosures)
cc: Ray Deyto, City of Stockton (w/o enclosures)
cc: Greg Gurss, GeoStrategies (w/o enclosures)
