<br /> 1. Describe the site location, the topography,prevailing wind directions, and nearby industrial or
<br /> populated areas. Provide information on the previous land use. Provide a site map which shows
<br /> property lines, buildings, treatment or storage ponds, sumps,piping, land application areas, and
<br /> surface water drainage courses within 1,000 feet of the site.
<br /> 2. Do active or abandoned industrial, agricultural, domestic, or groundwater wells exist on the
<br /> property or the land application area(s)? If yes,provide a map showing their location(s). Include
<br /> available data on the depth of groundwater and information on aquifers and aquitards identified
<br /> beneath the property.
<br /> 3. Describe all site activities which generate wastewater. Describe the average and maximum
<br /> volume of each waste stream discharged(i.e. tank cleaning,processing water, floor rinsing) as
<br /> well as combined volume discharge. Describe any daily or seasonal variations in volume and the
<br /> total discharged. Describe anticipated wastewater flow trends for the next ten years, and whether
<br /> it is anticipated that the flow will increase or decrease.
<br /> 4. Describe the quality of the wastewater and the quality of the treated wastewater. The description
<br /> should include maximum and average concentrations of BOD, suspended matter, total nitrogen,
<br /> total dissolved solids (fixed and volatile), and pH. Describe seasonal variations. If analytical
<br /> data is not available, estimates of wastewater quality can be provided with references.
<br /> 5. Provide a list of chemicals which are used in the processing activities, include all chemicals used
<br /> in disinfection,brining, storage, processing, etc. Describe any chemical additives that are added
<br /> to the process water or wastewater to precipitate dissolved solids, adjust hardness, prevent
<br /> scaling, biofouling, or other undesirable tendencies.
<br /> 6. Provide a description of the process supply water; at a minimum, the process supply water should
<br /> be characterized for the following analytical parameters: nitrate as nitrogen, total dissolved
<br /> solids, and pH.
<br /> 7. Provide a flow diagram of the wastewater generation and treatment system. Include a map
<br /> showing the locations of all treatment equipment, clarifiers, sumps, or pump stations. Describe
<br /> procedures to control contaminants in the wastewater streams and in the wastewater ponds.
<br /> 8. Describe opportunities to reduce, reuse, or recycle wastewater. Also describe the effect on
<br /> wastewater quality of recycling activities.
<br /> 9. Describe the storage/disposal of all stormwater and domestic wastewater. If septic systems are
<br /> used for disposal of domestic wastewater, include descriptions of the septic systems and the
<br /> maintenance of the systems. The descriptions shall include location, flow rate, size, and
<br /> construction materials, as well as leachfield size and location.
<br /> 10. If surface impoundments will be proposed to store wastewater, provide diagrams and a
<br /> description of the planned impoundment, location, size and depth, construction materials, inlet
<br /> and outlets, depth to groundwater below the bottom of the pond, and existing or planned
<br /> groundwater monitoring wells.
<br /> 11. Allowing for two feet of freeboard, what is the approximate storage capacity of the wastewater
<br /> pond? Provide a water balance which evaluates the containment structures' ability to provide
<br /> sufficient capacity on a month by month basis. Include monthly evaporation, precipitation, and
