In March, 1989,three (3) underground storage tanks.were removed without inspection and <br /> monitoring well seven (7) destroyed without a well^destruction permit. According to the <br /> approved tank removal plan, submitted by Petro-Check, Incorporated, American <br /> Environmental Management Corporation was contracted to perform laboratory analysis on <br /> samples taken, under chain of custody, at the time of the tank removal. Please submit a <br /> map indicating where the samples were taken, the chain of custodies accompanying <br /> samples, and the results of analysis. <br /> 5. Additionally, at some time, the tank pit was backfilled. Please provide information as to <br /> when it was backfilled and the type of backfill used. ' <br /> 6. Plate one (1) of the PAR was a map dated January 1988. This map indicated the presence <br /> of MWA. Upon a site inspection conducted April 9, 1991,.this well was not located. MWA <br /> is not being regularly sampled. Please provide further information regarding this well. <br /> 7. Sampling procedures, outlined in the LUFT Manual,xequiring that samples be stored and <br /> transported at 40C in an ice chest, must be adhered to. In the Field Procedures section of <br /> the May, 1989 Sampling-Monitoring Well Report, it was stated that samples were stored in <br /> a cooled ice chest. Please be informed that field procedures must be altered to reflect LUFT <br /> sampling procedures. Furthermore, be informed that'48 hours notice must be given to this <br /> office prior to any site activity, this includes sampling. <br /> Monitoring should be initiated pursuant to Section 2652(d) of the Underground Storage Tank Regulations <br /> (California Administrative Code, Title 23, Subchapter 16). The monitoring report shall be submitted to <br /> PHS/EHD and Central Valley Regional Water Quality Control Board (CVRWQCB) until the investigation and <br /> cleanup of the site is deemed adequate. The report is to show the: . <br /> -status of investigation and cleanup activities including the results`of all investigation completed to <br /> date; j <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented,.schedules for the completion of the investigation of <br /> the site and remediation; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -a site map showing the "zero Iine" of contamination, if known, and changes in the above <br /> measurements over the last quarter. <br /> Pursuant to Section 280.67, Subpart F of 40CFR of the Federal Register, PHS/EHD, will conduct a public <br /> notice to consider comments.on your Final Remedial Plan/Corrective Action Plan. <br /> .j <br />
