(Ultramar), Horizon Environmental Inc. (Horizon) is submitting <br /> this report documenting the destruction of 22 wells associated with the subject site located in <br /> San Joaquin County as shown on the enclosed Figure 1. The site was qualified for well <br /> destructions by the San Joaquin County Environmental Health Department (EHD) in <br /> response to a 2 December 2011 No Further Action Required Concurrence letter from the <br /> Central Valley Regional Water Quality Control Board. Horizon submitted Well Destruction <br /> Work Plan and Permit Applications (Work Plan) dated II January 2012. The EHD <br /> conditionally approved the Work Plan and issued the permits on 25 January 2013. Copies of <br /> the EHD permits are enclosed. <br /> Fieldwork Planning <br /> Wells associated with the subject site were located on four parcels as shown on the enclosed <br /> Figure 2. With the exception of well MW-2, the wells are located on parcels APN 094-030- <br /> 11, APN 094-030-12 and APN 094-030-13 owned by Shadrall Hammer Lane LLP <br /> (Shadrall). Well MW-2 was located on parcel APN 094-030-43 owned by 99¢ Only Stores <br /> (99¢ Only). Ultramar secured right-of-entry (ROE) agreements from Shadrall and 99¢ <br /> Only. Horizon retained All Well Abandonment (AWA) of Sacramento, California (C-57 <br /> License No. 848359) to provide well destruction services. The EHD issued the following <br /> permits (one per parcel): SR# 66455; SR# 66458; SR# 66459 and SR# 66460. <br /> Horizon updated the Site Health and Safety Plan to include well abandonment activities and <br /> notified Underground Services Alert (USA) to locate and mark locations of underground <br /> utilities. Property owners and EHD personnel were notified in advance of the scheduled <br /> fieldwork. Well destruction activities were conducted in accordance with the California <br /> Well Standards Bulletins 74-81 and 74-90 (California Well Standards), the EHD Well <br /> Standards, and approved exceptions as stated in an EHD 6 l"ebruary 2012 letter.
