PHS/EHD has a contract with the <br /> California State Water Resources Control Board to conduct a UST corrective action program. <br /> The above referenced site was placed in the PHS/EHD Local Oversight Program (LOP) in June <br /> 1993. <br /> On November 5, 2001, EHD staff met with you and Kurt Balasek from Wallace,Kuhl and <br /> Associates. We discussed the former UST investigation and cleanup that has taken place from <br /> 1993 to present. In order to move this site forward towards closure, you are directed to complete <br /> the following items: <br /> 1. One additional round of groundwater monitoring and sampling must be performed. <br /> At a minimum, monitoring wells 1,2,3, and 4 need to sampled and analyzed for TPH- <br /> gas, TPH-diesel, BTEX, 5 oxygenates, 1,2-DCA, and EDB. <br /> 2. On the day of our meeting, monitoring well 7 could not be found. It must be located <br /> and made accessible for monitoring. <br /> 3. The dust control well near the former USTs needs to be destroyed under EHD permit <br /> and inspection. Please submit a permit application, work plan, and fees. <br /> 4.
