Also, does SJCEHD have instructions/template on how to complete the MFR?. <br /> We received a letter yesterday from the CRWQCB concurring with SJCEHD closure <br /> recommendations, please see attached. Please verify that this is the correspondence you are <br /> looking for and if we could proceed with the well destruction project. <br /> We plan on submitting the well destruction work plan along with the permit applications, check, and <br /> access agreements. The City of Stockton encroachment permit for three right-of-way wells would <br /> not be ready as we plan on submitting this at the same time as the well destruction permit <br /> applications—will SJCEHD be able to process the permit application without the encroachment <br /> permit? <br /> Lastly, please confirm that the following methodologies are acceptable to SJCEHD for destroying <br /> the 23 wells. We plan on including these on our work plan. <br /> Based on the San Joaquin County Well Standards section 13.17.6: "If the well construction is <br /> unknown and/or is located in an area of known or suspected pollution or contamination, the well <br /> shall be destroyed by removing all material within the original borehole (including the well casing, <br /> screen, filter pack, and annular seal); and the created hole filled completely with appropriate sealing <br /> material." <br /> Based on the 2Q11 QMR (see attached), MW-4, MW-7, and MW-9 are wells with MTBE above the <br /> laboratory MRL (with only MW-9 above the 13 ug/L MCL). MTBE for the rest of the monitoring wells <br /> are below the laboratory MRL. Additionally, injection wells OS-1 through OS-10 are located in the <br /> area where the MTBE plume is not suspected. Considering these, the following is proposed: <br /> 1. Pressure-grout casing, air-knife to 3 ft bgs (section 13.17.12), cut casing to 3.5 ft bgs <br /> (section 13.17.12), form a mushroom cap (section 13.17.13), backfill with clean soil (section <br /> 13.17.14), and resurface borehole to match existing surface for wells MW-1 R, MW-21R, MW- <br /> 3 through MW-8, MW-10, MW-11, MW-13, VW-1, and OS-1 through OS-10. <br /> 2.
