Weber Avenue <br /> Stockton, California 95202-2708 <br /> RE: Work Plan for the Destruction of Groundwater Monitoring Wells <br /> Location: Quik Stop Market No 132,3555 West Hammer Lane,Stockton CA <br /> Dear Ms. McCartney: <br /> ATC Associates Inc. (ATC) is pleased to provide this workplan to San Joaquin County for abandoning <br /> two groundwater monitoring wells at the above-referenced address in Stockton, California. This <br /> workplan provides the scope of work, scheduling and fee information required to complete the task. <br /> is <br /> SCOPE OF WORK <br /> Two damaged groundwater monitoring wells are to be drilled out and sealed and a well destruction report <br /> is to be written and delivered to the San Joaquin County Environmental Health Department (SJCEHD) <br /> and the California Regional Water Quality Control Board (RWQCB). The two existing monitoring wells <br /> located on an adjoining property were damaged during site redevelopment activities. ATC is proposing <br /> I the proper abandonment of these wells by direction of the SJCEHD. ATC's field practices will meet or <br /> exceed the guidelines as stated in the ASTM Standard Guide for Decommissioning Groundwater <br /> Monitoring Wells(D5299-99). The following narrative describes the proposed approach to this project. <br /> PRE-FIELD ACTIVITIES <br /> Upon approval of this workplan, ATC will submit permit applications for monitor well abandonment to <br /> 1 the SJCEHD. In addition to this, ATC will prepare a site-specific Health and Safety Plan (HASP) in <br /> accordance with Title 8, Section 5192 of the California Code of Regulations. The HASP will contain a <br /> list of emergency contacts, a route map to the nearest hospital or emergency facility, and <br /> recommendations for Personal Protective Equipment (PPE) to be used during drilling. In an effort to <br /> uncover one of the damaged wells, ATC will also do a site reconnaissance and hand excavation in order <br /> to determine the exact location of the well which at this time is currently unknown. <br />
