SECOR <br /> Mr. Mike Infurna, <br /> -._ July 15, 2005 <br /> Page 2 <br /> PROPOSED MODIFIED SCOPE OF WORK <br /> The proposed scope of work consists of the installation of well MW-22 at the alternative <br /> location proposed on Figure 1. As proposed in the initial work plan and work plan <br /> addendum, the well will be completed at a depth of 22 feet bgs. A summary of the planned <br /> field work is presented below. Field and laboratory procedures for the activities described <br /> below are provided in Attachment 1. <br /> • Health and Safety Plan (HASP). As required by the Occupational <br /> Health and Safety Administration (OSHA) Standard "Hazardous Waste <br /> Operations and Emergency Response" guidelines (29 CFR 1910.120), <br /> and by the California Occupational Health and Safety Administration <br /> (Cal-OSHA) "Hazardous Waste Operations and Emergency Response" <br /> guidelines (CCR Title 8, Section 5192), SECOR will update the current <br /> site-specific HASP prior to the commencement of fieldwork. The HASP <br /> will.be reviewed by the field staff and contractors before beginning field <br /> operations at the site. <br /> • Permitting. SECOR has obtained a well construction permit from the <br /> SJCEHD. ConocoPhillips has obtained a right-of-entry agreement with <br /> the current owner of the Holiday Inn property (Indraprat Enterprises), <br /> who will be contacted prior to the beginning of field work activities. <br /> • Borehole Clearance Activities. The location of the boring will be <br /> marked, and Underground Service Alert (USA) will be notified at least 48 <br /> hours prior to drilling. Prior to the initiation of machine drilling, the <br /> proposed boring location will be resurveyed by a private utility locator to <br /> reconfirm the location of the nearby utilities. The borehole will be hand- <br /> augered to a depth of 5 feet bgs to clear the location of utilities in the <br /> area of the proposed boring location. <br /> • Soil Sampling and Analysis. The soil boring will be drilled by <br /> advancing eight-inch diameter augers to the proposed depth of 22 feet <br /> bgs. Soil samples for lithological description and chemical analysis will <br /> be collected at approximate 5-foot intervals. At least one soil sample <br /> from the boring will be selected for chemical analysis, and will be sent <br /> under chain-of-custody documentation to Severn Trent Laboratories, Inc. <br /> (STL). The soil sample(s) will be analyzed for the presence of gasoline <br /> range organics (GRO); benzene, toluene, ethylbenzene, xylenes (BTEX); <br /> and fuel oxygenates methyl tertiary butyl ether (MtBE), di-isopropyl ether <br /> (DIPE), tertiary amyl methyl ether (TAME), ethyl tertiary butyl ether <br /> (EtBE), tertiary butyl alcohol (TBA), ethanol; and lead scavengers 1,2- <br /> dichloroethane (1,2-DCA) and ethylene dibromide (EDB) by <br /> Environmental Protection Agency (EPA) Method 8260B. <br /> --i <br /> 11195 Second Addendum to 9-04 WP.doc SECOR lntemationallncorporaied <br />
