(NOA)has recently completed and submitted a"Monitoring <br /> Well Installation Report,February 10, 2005"for the above referenced facility. A review of that <br /> document will reveal a problem with monitoring well MW-3. Recent sampling revealed a persistent <br /> sedimentation problem and an anomalously high pH and Total Dissolved Solids (TDS) content in <br /> that well. The elevated pH and TDS content is characteristic of grout contamination. The purpose <br /> of this letter is to request your approval for installing a replacement well for MW-3. <br /> As you know, a Quarterly Groundwater Monitoring Program is being initiated at the site beginning <br /> the first quarter 2005. It is our intent to immediately install the replacement well, such that it can be <br /> included in the quarterly monitoring program. <br /> The replacement well for MW-3 will be referred to as MW-5. The attached map shows the proposed <br /> location for MW-5. Groundwater elevation contours developed from recent sampling data at the <br /> site are also shown on the attached map. Please note that the proposed location for MW-5 is <br /> immediately down-gradi ent of the wastewater pond based on the recent data shown in the attached <br /> map. Well installation procedures for MW-5 will be those identified in the "Groundwater Quality <br /> Investigation Workplan". That is the same workplan that was used to install MW-1 through MW-4 <br /> and that was approved for use by the Regional Water Quality Control Board in a letter dated 19 <br /> October 2005. That plan describes the procedures for drilling, soil sampling,well construction, <br /> well development,and professional survey of location and elevation. It is anticipated that the first <br /> quarterly sampling event will be conducted two-weeks after installation of MW-5. <br /> It is proposed that well MW-3 be abandoned at the same time that well MW-5 is installed. Prior to <br /> abandonment, an attempt will be made to remove all sediment from the base of MW-3.
