Ms. Kathy Amaru Page 2 of 4 <br /> Regional Water Quality Control Board-Central Valley Region <br /> November 18,2010 <br /> PERMITTING <br /> The SJCE14D requires a permit for the destruction and installation of monitoring wells. The appropriate <br /> permit applications and fees will be submitted before conducting Feld activities. <br /> Following the receipt of a drilling permit ftom the SJCEHD, the field activities will be conducted <br /> according to the scope of work presented in this work plan. Your office will be notified a minimum of <br /> 48-hours prior to commencement of Feld activities. <br /> PROJECT HEALTH AND SAFETY <br /> A site-specific HASP will be prepared for the subject site, which will be available on site during field <br /> activities. The plan will be reviewed by the consultant's field personnel and subcontractor staff before <br /> the start of field activities. <br /> UTILITIY CLEARANCE <br /> Well locations will be clearly marked and Underground Service Alert (USA) will be notified at least 48 <br /> hours before field activities begin. <br /> WELL ABANDONMENT (MW-23-SP) <br /> Groundwater monitoring well MW-23-SP will be properly abandoned by a California licensed drilling <br /> contractor,under the direction of a geologist. The well location is presented on Figure 1. <br /> The well will be abandoned by over-drilling to its total depth of 21 feet bgs with 10-inch, OD, hollow- <br /> stem augers and tremie grouted with a Portland cement grout containing less than 5% bentonite powder <br /> to the surface. The boring log with well construction diagram is presented as Attachment A. <br /> MONITORING WELL INSTALLATION (MW-23R) <br /> URS will install one groundwater monitoring well (MW-23R) to replace the abandoned well MW-23. <br /> This replacement well will be installed approximately 10 feet from the previous well location. See <br /> Figure 1. <br /> The monitoring well will be drilled using a 8.25-inch OD hollow stem auger equipped drilling rig. The <br /> well will be constructed using 2-inch-diameter, flush-threaded, schedule 40 PVC well casing. Fifteen <br /> feet of 0.010-inch, machine-slotted well screen will be set from approximately 6 feet to 21 feet bgs. <br /> Depth to groundwater during the 3Q10 sampling event was approximately 5.5 feet bgs.
