The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection appointments (a <br /> 48-hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Verification of leak monitoring communication for all installed vacuum, pressure, or <br /> hydrostatic (VPH) components in under dispenser containment sumps (UDCs) and fill <br /> sumps; communication at end point(s) of each vacuum zone termination must be verified. <br /> All appropriate connections to facilitate communication testing must be present. <br /> b. VPH components installation verification (shroud boots for spill containers, band clamps for <br /> spill containers, fill, vent and STP sump rubber gaskets, etc.). <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 1868 E.
