Current financial responsibility documents have not been uploaded into the California Electronic <br /> Reporting System(CERS). The most recent copy submitted to EHD was dated 8-1-13. Current financial <br /> responsibility documents are required to be submitted annually. Immediately submit a copy of the <br /> financial responsibility documents to the EHD. Log into CERS, upload the required information, and <br /> submit for review by the EHD. Complete by 9-25-14. <br /> 105. UST Information for the 91 and diesel tanks are not current in CERS. Tank primary containment <br /> should list fiberglass. Any change of information must be updated in CERS within 30 days of the <br /> changes. Immediately log into CERS, update the required information, and submit for review by the <br /> EHD. Complete a separate tank form for each tank. <br /> 106. The monitoring plan in CERS is neither current nor approved by the EHD. The monitoring panel <br /> model for the USTs throughout the plan should list TLS-350R. The tank leak sensor model # should list <br /> 303 for all three tanks. The Under Dispenser Containment monitoring for all USTs should list "NO" for <br /> stopping flow of product at the dispenser. The diesel UST should list leak sensor model # 208 (304) in <br /> the section entitled Pipe Monitoring Is Performed Using The Following Methods. The monitoring and <br /> response plans must be current and approved by the EHD. Immediately log into CERS, update the <br /> required information, and submit for review by the EHD. Complete a separate record for each tank. <br /> Maintain an approved copy on site. <br /> 107. The plot plan/site map has not been uploaded into CERS. The plot plan/site map which shows <br /> where the monitoring is performed must be uploaded into CERS.
