The designated operator failed to document all the alarms from the attached alarm history on the <br /> 6-25-14 and 8-2913 designated operator monthly inspection reports and failed to document that they <br /> were responded to appropriately. The missing alarms include: 6-11-14 L20 Disp. 5/6 annular fuel alarm <br /> and 8-1-13 Q3 diesel PLLD shutdown alarm. The designated operator failed to document responses to <br /> the 11-26-13 Q3 PLLD shutdown alarm, 12-20-13 Disp. 7/8 annular fuel alarm, 3-24-14 Q1 87 PLLD <br /> shutdown and gross line failure alarms, 11-2-13 Q3 diesel PLLD shutdown alarm, and 10-23-13 Q2 91 <br /> PLLD shutdown and gross line failure alarms. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response <br /> to any alarms to the monthki report. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> 303. The 87 turbine sump sensor was resting on a hose which prevented the sensor from touching the <br /> bottom of the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment <br /> shall be maintained to be able to detect a leak at the earliest possible opportunity. During the inspection, <br /> the technician repositioned the sensor to the lowest part of the sump to detect a leak at the earliest <br /> opportunity. The hose was moved aside. <br /> 404. The vent sump lid is not liquid and vapor tight. A small amount of liquid was found in the fill sumps <br /> of the 87, 91, and diesel USTs. Secondary containment shall be constructed to prevent any water <br /> intrusion into the system by precipitation, infiltration, or surface runoff. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage <br /> accordingly. Immediately contact a properly licensed, trained, and certified contractor to address the <br /> water intrusion into the UST fill sumps and make the vent sump liquid and vapor tight under permit and <br /> inspection of the EHD. <br /> 999. HSC 25404(e)(4) Electronic submittal requirement on the California Electronic Reporting System <br /> (CERS) has not been met. Facility shall submit electronic information for hazardous materials <br /> management system, including Hazardous Waste and UST on the CERS website which can be found at <br /> http://cers.calepa.gov/.
