PUBLIC HEALTH SERVICES
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue,Third Floor • Stockton, CA 95202
209/468-3420
JUL 312001
TONY ARNAIZ
CENTRAL VALLEY CONSTRUCTION
P O BOX 830
LATHROP CA 95330
RE: 12565 Manthey Road
Lathrop, CA
San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD)
has received a copy of the letter to you from the Central Valley Regional Water Quality
Control Board (CVRWQCB) concerning a work plan that proposed installation of
monitoring wells at the above referenced site. San Joaquin County has a well ordinance
and well standards, which are enforced by PHS/EHD. A well permit is required prior to
digging, drilling, boring, driving, repairing or destroying any well whether it is used for
domestic, irrigation, testing, geophysical, seismic, subsurface boring, monitoring,
injection, extraction, vapor probe, cathodic protection or other purposes. A well permit
application must be submitted to PHS/EHD for approval by a C-57 licensed well
contractor or an authorized representative with an $89 permit fee. In addition, PHS/EHD
has to charge for staff time used for to approve well permits and inspect well grouts.
The current hourly rate is $89 per hour.
Enclosed is a "Master File Record" form that is used for PHS/EHD billing purposes to
cover the hourly rate for staff time spent on your site. This form must be completed and
submitted to PHS/EHD with $261 (covers first 3 hours of staff time) before we will
evaluate or approve any required well permits for work proposed at your site.
Please send a copy of the work plan approved by CVRWQCB to PHS/EHD so the
proposed construction of the monitoring wells can be evaluated pursuant to our well
standards.
If you have any questions contact me at (209) 468-3449.
Donna Heran, REHS, Director
Environmental Health Division
<.J
Margaret Lagorio, Supervising REHS
Unit IV
Enc.
c: John Cummings &Associates
A Division of San Joaquin County Health Care Services
