f I <br /> 9000562C, Page 7 <br /> C <br /> B.5.2.6 All personnel (WCC and subcontractor) shall have available upon request <br /> proof of respirator fit test. This document must state the individual's name, manufacturer, <br /> model and size of respirator for which the employee has been successfully fitted. This <br /> document must also have the date of fit, and this date must be within the past 12 months. <br /> Individuals failing to provide these documents or those whose respirator fit test has not <br /> been conducted within the past 12 months will not be permitted to work on site. <br /> B.5.3 Decontamination <br /> B.5.3.1 Personnel <br /> B.5.3.1.1 Personnel engaged in soil sampling shall decontaminate according to <br /> Operating Procedure HS-512, Personnel Decontamination. <br /> B.5.3.2 Equipment <br /> B.5.3.2.1 Contaminated hand tools must be decontaminated (water wash) before <br /> removal from the site. Sampling equipment may be decontaminated by the method used to <br /> prevent cross-contamination of samples. <br /> B.6 GENERAL HEALTH AND SAFETY REQUIREMENTS <br /> B.6.1 Medical Clearances <br /> B.6.1.1 Employees of WCC and its subcontractors assigned to perform work in the <br /> restricted area must have medical clearances. The only acceptable proof of clearance is a <br /> letter or document, signed by a physician, certifying that the physician performed an <br /> examination within the past 12 months and found the person physically fit to wear a <br /> respirator and perform work at hazardous waste sites. WCC personnel perofrming less <br /> than 30 days of field work at hazardous waste sites (low to moderate hazards) may have <br /> biennial physicals (24 months), based on the reveiwing physician's recommendations. <br /> Documentation of medical clearance shall be furnished upon request. Individuals failing to <br /> provide these documents or whose medical clearance is not current will not be permitted to <br /> j' work on site. <br />
