The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Denny's as of February 10, 2020. <br /> Open violations from August 14, 2019 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for carbon dioxide was not complete. No value was listed for the average <br /> daily amount. Pleases change the max daily amount to say 420 pounds, the largest container size to say 450 <br /> pounds and the average daily amount to say 400 pounds. <br /> This information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The local Untidied Program Agency(UPA) phone number <br /> was not provide and the locations of the marked items in section G was not identified. Please add 209-468-3420 as <br /> the UPA phone number on line C3 and where things are located in section G. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 2 <br />
