4.7 EMERGENCY MEDICAL ASSISTANCE <br /> The nearest emergency medical facility selected to support this site is <br /> Organization: San Joaquin County General Hospital <br /> Contact: Emergency Medical Services <br /> Address and Location: 500 West Hospital Road <br /> French Camp, Ca 95231 <br /> Telephone: (209)468-6818 <br /> A map to this facility is located in this document(Figure 4-2) <br /> 5.0 MEDICAL SURVEILLANCE <br /> (in compliance with 29CFR 1910.120(f) and other substance-specific medical surveillance requirements <br /> found in 29CFR 1910.1001-1052) <br /> The medical surveillance section of the HASP describes how worker health status is monitored <br /> at this site. Medical surveillance is used when there is the potential for worker exposure to <br /> hazardous substance at levels above OSHA permissible exposure limits or other published <br /> limits. The purpose of a medical surveillance program is to medically monitor worker health <br /> to ensure that personnel are not adversely affected by sire hazards. The provisions for medical <br /> surveillance at this site are based on the site characterization and job hazard analysis found in <br /> chapter 3 of this HASP and are consistent with OSHA requirements in 29 CFR 1910.120(f) and <br /> the following substance-specific requirements: ARSENIC METAL AND INORGANIC <br /> COMPOUNDS,as,AS(1910.1018, 1926.1118). <br /> The medical surveillance program is consistent with 29CFR 1910.120(f) and addresses the <br /> following information: <br /> • Provisions of the site medical surveillance program <br /> • Communication between the site, physician, and workers <br /> • Medical recordkeeping procedures <br /> The person responsible for ensuring the implementation of this program is Tyson Fulmer <br /> 6.0 PERSONAL PROTECTIVE EQUIPMENT <br /> (in compliance with 29CFR 1910.120(b)(4)(ii) and 29CFR 1910.120(g)) <br /> This chapter of the HASP describes how personal protective equipment(PPE) is used to protect <br /> against employee exposures to hazardous substances and hazardous conditions on this site. <br /> Environmental Risk Services,Corp. <br />
