Obtain one well destruction and one well installation permit from PHS/EHD and notify PHS/EHD at <br /> least 48 hours prior to the proposed scheduled work dates. <br /> 3. Sampling of excavation pit should be adequate to calculate remaining mass and demonstrate areas that are <br /> non-detect. PHS/EHD recommends 10 to 12 soil samples. <br /> 4. Excavation back-fill material should include fines. Propose material to PHS/EHD prior to ordering. <br /> 5.
