The installation of at least two monitoring wells, one deep well ( minimum 40' total depth) for vertical <br /> definition and one shallow well ( 20' total depth) to replace MW-3. <br /> 4. Laboratory analysis of soil and groundwater samples for total petroleum hydrocarbons, BTEX, and fuel <br /> oxygenating compounds. <br /> COMMENTS AND CONDITIONS: <br /> I. One additional deep screened monitoring well ( minimum 40' total depth) may be needed for <br /> conformation of vertical definition. <br /> 2. Obtain one well destruction and one well installation permit from PHS/EHD and notify PHS/EHD at <br /> least 48 hours prior to the proposed scheduled work dates. <br /> 3. Sampling of excavation pit should be adequate to calculate remaining mass and demonstrate areas that are <br /> non-detect. PHS/EHD recommends 10 to 12 soil samples. <br /> 4. Excavation back-fill material should include fines. Propose material to PHS/EHD prior to ordering. <br /> 5.
