Remedial Action Pl"a <br /> Frontier Transportation <br /> I <br /> June 17, 2004 <br /> j Page it <br /> 7.2) Chain-of-Custody <br /> The objective of the Chain-of-Custody Program is to allow the tracking of <br /> possession and handling of individual samples from the time of field collection through <br /> laboratory analysis. Once a sample is collected, it becomes part of the Chain-of-Custody <br /> process. A sample is "in custody" when (1) it is in someone's possession; (2) it is within <br /> visual proximity of that person; (3) it is in that persons possession, but locked up and <br /> -.� sealed (e. g. during transport); and (4) it is in a designated secure sample storage area. <br /> 7.2.1) Chain-of-Custody Record and Request for Analysis Report <br /> Chain-of-Custody records establish the documentation necessary to <br /> trace sample possession from the time of collection to analysis. A serialized Chain-of- <br /> Custody and Request for Analysis Report will be completed and will accompany each <br /> L batch of samples. The record will contain the following information. <br /> * Project name and number; <br /> * Request for Analysis control number (for cross reference); <br /> * Names of sampling team members; <br /> I J * Laboratory destination; <br /> J * Carrier/waybill number; <br /> * Sample number; <br /> * Sample location and description-, <br /> * Date and time collected; <br /> l * Sample type; <br /> I i * Container type; <br /> * Special instructions; <br /> * Possible sample hazards; <br /> * Signatures of persons involved in the chain-of-possession. <br /> When sample custody is transferred to another individual, the <br /> ' _J samples must be relinquished by the present custodian and received by the new <br /> custodian. This will be recorded at the bottom of the Chain-of-Custody Record and <br /> Request for Analysis Report where the persons involved will sign, date and note the time <br /> of transfer. During field operations, each project geologist will act as the custodian for <br /> the samples he or she collects . Samples will not be left unattended unless placed, along <br /> with the Chain-of-Custody Record, in a secure container. <br /> - - The Chain-of-Custody Record and Request for Analysis Report is <br /> j a multi-part form that allows the record to be kept in duplicate. One copy will <br /> accompany the sample shipment to the laboratory and one copy will be kept with the <br /> field logbook. All documents that accompany shipments will be enclosed in zip-lock bag <br /> and taped to the inside top cover of the shipping container. <br />
