This also furnishes further evidence that the Chain-of-Custody is complete. <br /> The form will contain the following information: <br /> * Cross-reference to the Chain-of-Custody Record; <br /> i * Project name and number; <br /> j * Sample number; <br /> * Sample volume; <br /> * Preservative as required; <br /> * Requested testing program; <br /> * Required turnaround time; <br /> + Possible hazard identification; <br /> * Sample disposal requirements; <br /> The form will be signed and dated by the receiving laboratory <br /> sample management custodian. <br /> 7.2.2) Sample Identification <br /> Sample labels prevent the misidentification of samples. Following <br /> sample collection; labels will be affixed to each sample container.
