An accurate UST Monitoring Site Plan was not submitted, ensure that the submitted map identifies dispenser locations and monitoring/sensor type and locations and their monitoring. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a Class II violation. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 5-15-2014 and a test report was submitted on 7-18-2014 , and was 1 month past due. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 114 CCR 2636(f) Failed to perform 0.1 gph annual line test for pressurized piping without fail safe or shut down. The 87-octane turbine was not fail safe when tested and does not meet the requirements for annual line tightness testing exemption. The service technician replaced the malfunctioning turbine relay and verified that this turbine has positive shut down and is fail safe, which satisfies the annual line tightness testing requirement. Ensure that all leak detection equipment is properly programmed at all times. This was corrected on site. This is a Class II violation. 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. New designated operators conducted monthly designated operator inspections and the notification was not provided to the EHD within 30 days of the change. Ron Rocha, Ruben Galindo and Jose Ochoa conducted designated operator inspections during the 2014-2015 years and were not listed on the designated operator statement submitted into CERS. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility.
