The monitoring system certification date <br /> on the July 16, 2014 inspection report listed 5-16-2014 instead of 5-15-2015. The monitoring system certification date <br /> on August 2014 through April 2015 reports, listed 5-20-2014 instead of 5-15-2015. The monitoring system certification <br /> date on the May 27, 2015 inspection report was listed as 5-15-2015 instead of 5-28-2015. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 87-octane sump sensor was raised approximately 3 inches up the sump saddle and off the lowest point of the <br /> sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be <br /> able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by <br /> the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the UDC 3/4 sump failed to stop the flow of product at the dispenser when tested. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. <br /> This is a Class II violation. <br /> 310 CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br /> The 87-octane sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring <br /> of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br /> flow of product at the dispenser when it detects a leak.
