The permit has been approved with the following conditions: <br /> 1. Contact the Local Fire Department, Local Planning or Building Department, and <br /> the San Joaquin Valley Air Pollution Control District for any applicable permits <br /> and requirements. <br /> 2. Contact Muniappa Naidu at (209) 468-3439 to arrange the following inspection <br /> appointment(a 48-hour notice is required for inspection appointments): <br /> a. Removal of UDC and Sampling(See Table#2), If water is encountered <br /> water sample will have to be taken. <br /> b. Primary piping soap/pressure test. <br /> c. Secondary piping soap/pressure test. <br /> d. UDC lake test. <br /> e. To ensure that monitoring has been restored to meet the requirements of <br /> California Code of Regulations Title 23 and the Health & Safety Code (A <br /> cold start will be required). <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. Individuals installing or repairing the UST Systems or components shall possess <br /> or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current International Code Council (ICC) UST <br /> Installer/Retrofitting certificate and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being installed. <br />
