Qualified service technicians shall possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council(ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested. <br /> d. Each individual installing or testing any component shall hold a certificate <br /> of training from the manufacturer of that component. <br /> 6. Complete and submit a copy of a Underground Storage Tank Certification of <br /> Installation/Modification (Former Form C)to this office no later than 30 days <br /> after work is completed. <br /> 7. Contractor must provide information regarding the conversion adaptor frame from <br /> the dealer as soon as the frame is purchased. <br /> 8. The technician must print out a copy of the alarm history prior to performing the <br /> "cold start"of the Veeder Root system and have the copy available at the time of <br /> EHD inspection for review. <br /> 9. A secondary containment testing must be performed for the entire tank system <br /> once it is in operating condition. <br /> 10. Permit expires 180 days from date of approval.
