All serious injuries call 911. <br /> Contact Project PM and Regional Safety Manager immediately after site is secure. <br /> For all minor injuries (ex. minor cuts and bruises) immediately contact your local human resources official for the local <br /> worker compensation doctor. <br /> For any injury that occurs on the job notify the doctor that the injury is a workers compensation injury. <br /> Emergency Equipment <br /> Cellular phone (or verify immediate access to immediately available landline) <br /> First aid kit (10 person minimum) must meet or exceed OSHA 29CFR1910.151 <br /> Fire extinguisher(s) 5 lbs. rated at least 2A and 56C (serviced annually and inspected monthly with inspection tag <br /> attached) <br /> Eyewash bottle if adding water to pre-preserved containers <br /> Fire blanket <br /> Emergency Reporting <br /> Per Table 1, Incident Notification Matrix, the SHSO will immediately report injuries or illnesses, vehicle <br /> accidents, releases (hazardous material, hazardous waste, or uncharacterized waste), inspections by <br /> regulatory agencies, and any incident that could reasonably have caused a significant injury or property <br /> damage (fire, catastrophic equipment failure, capsized heavy equipment, etc.). The SHSO will contact <br /> emergency response organizations (if needed) and the PM. The PM will notify the Division Manager, <br /> Program Manager, ECHS Manager and the client (as appropriate). If the PM is not available, the SHSO <br /> will contact the SAIC Regional Alternate Program Manager or other PMs, as necessary. If the SHSO is <br /> not available, the FM will make the aforementioned contacts. See Site Health and Safety Hierarchy on <br /> page 8. See the SAIC Incident Notification Contact List and the EMC Emergency Reporting contact <br /> protocol in Table 1 and 2 of this HASP (also posted in the Emergency Reporting tab section). See also <br /> EC&HS Procedures 4.1, Accident Reporting and 24, Regulatory Agency Inspections and Incident <br /> Reporting for details. <br /> Emergency Evacuation, Assembly Points & Reporting Procedures <br /> Emergency evacuation of the site may be necessary under certain conditions. In the event that the site <br /> must be evacuated, the SHSO or other personnel will invoke a "Stop Work' authorization. One long blast <br /> from a portable air horn or vehicle horn will serve as the site emergency alarm. All work is to stop <br /> immediately. The SHSO or FM will call 911, the utility company (if necessary), safety personnel at <br /> adjacent facilities, and the PM. Workers will exit the site and proceed to the designated primary assembly <br /> point.
