Unifirst Corporation page 2
Work plan comment
COMMENTS AND CONDITIONS: (continued)
• The proposal included only quarterly sampling for one year.
California Code of Regulations, Underground Storage Tank
requirements (sect 2652d) require report submittal every three
months until the investigation and cleanup are complete. SJCEHD
will require monitoring well sampling reports to be submitted
quarterly and continued until notified otherwise.
• Monitoring well installation procedures included in the work plan
noted, "a small amount of potable water will be added (if necessary)
to facilitate efficient placement of the filter pack". SJCEHD will
require instead, that the PVC casing be `surge-blocked' to compact
the filter pack prior to setting the bentonite seal.
• Also included in the monitoring well construction details was the
emplacement of grout by "free-fall". Although SJCEHD does allow
grout placement typically in some soil borings by free-fall (not to
exceed 30-feet) experience has shown that free falling this heavy
material onto a soft bentonite seal of a monitoring well has created
problems with the well later on. SJCEHD recommends that the grout
be tremied pumped from the bottom-up instead of being emplaced
by free-fall from the surface.
• California Code of Regulations (CCR-section 2729.1) requires UST
investigative/remedial data to be electronically submitted to
Geotracker. Please have your consultant ensure that the laboratory
data, monitoring well survey (X & Y/Geo data) and the site/location
map are submitted to the SWRCB Geotracker database.
• Please submit a Well Permit Application Form and the $89 fee now.
REASONS FOR DISAPPROVAL OF THE WORKPLAN [H&S Section
25296.1 0(c)(3)1:
• none.
DATED: March 25, 2005 BY G S6
Michael Infurna, VLHS.
