West Valley Auto Spa, Inc. <br />Hazard Survey Report <br />April 13, 2012 <br />Page 2 <br />a building permit was issued, and in March of 2012, construction of the seismic anchorage was <br />completed. A copy of the construction documents is included as an attachment to this report. <br />The seismic anchorage of the tank consists of two components. First, the top of the tank is <br />secured to the exterior building wall by means of welded and bolted steel components. Second, <br />the base of the front of the tank is secured to the concrete floor slab by means of steel plates and <br />stainless steel expansion anchors. With all components installed, the tank is secured to the <br />building and protected from sliding, uplift and overturning as a result of seismic forces. <br />On March 28, 2012 and again on April 9, 2012, we conducted a subsequent visual observation <br />of the tank. We observed in installation of the seismic anchorage devices that we had designed, <br />including the steel bracing to the exterior wall, and the hold-down devices to the concrete floor <br />slab. All seismic anchorage devices appeared to be installed in compliance with our design. <br />Photographs of the tank and anchorage devices are included below. <br />Photoeraph of Entire Tank <br />
