c ii <br /> I. <br /> ATTACHMENT 3.4-INITIAL AND ANNUAL OIL HANDLING PERSONNEL TRAINING AND BRIEFING LOG <br /> (1).Read and review this entire Oil Handling Plan with each employee annually and upon initial hire(individually oras o group). <br /> (2).Keep a running log of each training event and the names of the attendees. <br /> (3).In order to keep this document up to date,hand write additional post script briefing notes with date of inception. <br /> (4).Note the date of briefing note incorporation into the text in the Addendum Log. <br /> (5).Archive the previous oil Handling Plans and the training logs behind the current Oil Handling Plan. <br /> (6).Retain all records for three years. <br /> (7).Review and recertify oil handling plan every five years. <br /> WEST VALLEY AUTO SPA, INC. <br /> OIL HANDLING PLAN <br /> December 13, 2013 <br /> A. EMPLOYEES ROLE <br /> The role of the employee is to perform each task in a clean,workman like manner being conscientious of <br /> procedures and observant of potential failures in service and methodology. The employee is to report <br /> observed potential failures to the Lube Shop Manager or the Facility Operator. The Lube Shop Manager <br /> and the Facility Operator will add updates/briefing notes to this oil handling plan. <br /> B. COMPANY HIERARCHY AND CONTACT LIST <br /> 1).See attached Table G-8 <br /> C. HOUSE KEEPING <br /> 1). Keep loose objects secure and stable so nothing can fall and damage oil lines or tanks. <br /> 2). Keep shop floor mopped and oil free. <br /> 3). Keep building envelope secure and alarms operational to prevent vandalism. <br /> 4). Keep roof leak free so not to cause tank corrosion. <br /> 5). Keep scratches in the exterior paint of the tank touched up to prevent corrosion. <br /> 6). Keep trolley pan rail troughs clear of tools and debris. Debris may cause wheels to ride up <br /> above the rail and dismount the rail. <br /> D. WEATHER <br /> 1). Keep the bulkhead holes covered or plugged. <br /> 2).Close the windward bay doors when rain is wind driven. <br /> E.TANK FILLING <br /> Secondary containment under the oil supply truck is not feasible. Therefore in order to limit the <br /> potential spill to just the volume of the hose (13gal), redundant observation by an individual with <br /> working knowledge of the truck's panic shut off procedure must be provided. A designated employee is <br /> to: <br /> 1). Introduce themselves to the oil delivery operator. <br /> 2). Inform the operator that they are the designated redundant observer incase of operator <br /> incapacitation. <br /> 3).Get quick training on the truck's panic shut off. <br /> 4). Clear access to the tanks by providing good housekeeping. <br /> 5). Look for missing bulkhead covers and plugs. <br /> 6).Stay focused on the task at hand. <br /> 7). Observe for and clean up any oil escapement. <br /> 8). Observe for over flows from an unattended nozzle. <br /> 25 of 33 <br />
