The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Secondary piping soap test: One hour pressure test per manufacturer's guidelines. If <br /> there are no manufacturer's guidelines, test shall be performed using an applicable <br /> method specified in an industry code or engineering standard. <br /> b. Secondary containment testing on any failed tanks and pipe runs. <br /> 4. If concrete will be broken near the any UST and piping, a secondary containment test <br /> must be performed on the affected UST and piping once concrete is poured in <br /> accordance with the manufacturer's guidelines. <br /> 5. Secondary containment testing for the failed components affected or installed are <br /> required six months after installation. Provide 48 hours notification to the EHD when this <br /> test is scheduled. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
