Lodi Avenue <br /> Lodi, San Joaquin County, California 95240 <br /> SECOR Proposal No. 15g-04-149 <br /> Dear Ms. Schulz: <br /> SECOR International Incorporated (SECOR) has prepared this proposal to complete a Phase II <br /> Environmental Site Assessment (ESA) at the Pizza Hut located at 21 W. Lodi Avenue in Lodi, San <br /> Joaquin County, California (subject property). SECOR understands that this project is of the utmost <br /> importance and that a written report is required by June 24, 2004. SECOR previously prepared a <br /> Phase I ESA, dated May 12, 2004 for the subject property. Preliminary findings of the Phase I ESA <br /> identified the following recognized environmental conditions: <br /> • The subject property operated as a gasoline station from at least 1961 until 1974. The final <br /> disposition of the underground storage tanks (USTs) is unknown. The potential presence of <br /> USTs at the subject property, and the former storage and dispensing of petroleum products <br /> at the subject property, represents a recognized environmental condition. <br /> Based on these findings, GE Franchise Finance has requested that SECOR prepare this proposal <br /> for a Phase II ESA to further assess the potential for subsurface contamination. <br /> PROPOSED SCOPE OF WORK <br /> Based on SECOR's previous Phase I ESA, SECOR proposes to conduct a Phase II ESA, consisting <br /> of a geophysical survey to assess for the presence of USTs formerly operated at the subject <br /> property, and a limited subsurface investigation at the subject property to assess the possible <br /> presence of impacted soil and groundwater. Specific details of the proposed work are as follows: <br /> Task 1 - Preparation of Site <br /> Prior to initiating sampling activities, SECOR will contact the public utility locating service to request <br /> that locations of subsurface utilities at the proposed boring locations be marked. At least three days <br /> should be allowed to identify utilities and clear boring locations. <br /> In addition, SECOR will also obtain drilling permits from the County Health Department and work <br /> with property management to gain access for completion of the proposed work. At least one week <br /> will be required to obtain drilling permits. <br /> SECOR will also prepare a site-specific Health and Safety Plan (HASP) to address potential <br /> environmental and physical hazards associated with the proposed sampling. The HASP will be <br /> reviewed by SECOR field personnel and reviewed with SECOR subcontractors prior to initiating the <br /> geophysical survey and field sampling activities.
