The following i an itemized list of hazardous waste violations that have not been <br /> addresse for Global Materials Management, LLC as of April 17, 2017. <br /> Open violations from August 26, 2016 inspection <br /> Violation #501 - Failed to completely label stationary hazardous waste tanks. <br /> SE-25 Oil filter Kruncher s seen storing hazardous waste (used oil) in the secondary containment. It was <br /> described by Mr. Kandathil the secondary containment was drained every 10-15 minutes into oil drums stored <br /> nearby. He stated approxi ately 3-5 drums were generated per shift. During the inspection the crusher was not <br /> running, nor was any oil re oved from the secondary containment. The secondary containment overflows once <br /> every couple of days. <br /> All hazardous waste tanks hall be marked with the words "Hazardous Waste" and the accumulation start date. <br /> Immediately ensure that al hazardous waste tanks are marked with the required information. <br /> Violation #502 - Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a to k assessment for the used oil tank under the filter crusher(Kruncher). The owner or <br /> operator of a hazardous w ste tank shall obtain a written assessment reviewed and certified by an independent, <br /> qualified, professional engi eer, registered in California attesting that the system has sufficient structural integrity, is <br /> acceptable for the transfer ng, storing, and treating of hazardous waste, and that the tanks and containment system <br /> are suitably designed. Im ediately have an assessment prepared for the used oil tank under the crusher. Submit a <br /> schedule showing when thi will be accomplished to the EHD. <br /> Violation#507 - Failed to onduct daily inspections of hazardous waste tank system and maintain records <br /> onsite. <br /> Inspections of the hazardo s waste tank system was not performed and documented. The following items are <br /> required to be inspected at east daily: <br /> 1. overfill/spill control eq ipment; <br /> 2. aboveground portions of the tank system to detect corrosion or releases of water; <br /> 3. data gathered from m nitoring and leak-detection equipment; <br /> 4. outside areas of the t nk system including secondary containment to detect erosion or signs of releases of <br /> hazardous waste; <br /> 5. for uncovered tanks, t e level of waste in the tank. <br /> Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. <br /> Submit a copy of the inspec ion log for the next 30 days to the EHD. <br /> Violation#509 - Failed to i mediately respond to spills and leaks from hazardous waste tank system. <br /> SE-25 Oil filter Kruncher wa seen storing hazardous waste (used oil) in the secondary containment. The include at a minimum: <br /> - spill prevention controls (e.g., check valves, dry discount couplings); <br /> -overfill prevention contr Is (e.g., level sensing devices, high level alarms, automatic feed cutoff, or bypass to a <br /> standby tank); and <br /> - maintenance of sufficie t freeboard in uncovered tanks to prevent overtopping by wave or wind action or by <br /> precipitation. Uncovered tan s shall be operated to ensure at least 60 centimeters(2 feet) of freeboard, unless the <br /> tank is equipped with a cont inment structure (e.g., dike or trench), a drainage control system or a diversion <br /> structure (e.g., standby tank with a capacity that equals or exceeds the volume of the top 60 centimeters(2 feet) of <br /> the tank. <br /> Immediately implement app priate controls and practices to prevent spills and overflows from the tank or <br /> secondary containment syst ms. <br /> Page 6 of 8 <br />
