The following ian itemized list of hazardous waste violations that have not been <br /> addresse I <br /> for Global Materials Management, LLC as of April 17, 2017. <br /> Open violations from August 26, 2016 inspection <br /> Violation #304 -Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> The facility was overwhel ed with 55 gallon drums and -300 gallon totes inside the building and outside. On the <br /> South side of the building t ere were over two hundred 55-gallon drums of used oil, hazardous waste, used oil <br /> filters, and misc. It was di icult at times for inspectors to maneuver between drums. Adequate aisle space shall be <br /> maintained to allow the un bstructed movement of personnel, fire protection, spill control equipment, and <br /> decontamination equipme t to all areas of facility operation in an emergency. Immediately maintain aisle space to <br /> allow the unobstructed mo ement of personnel, fire protection, spill control equipment, and decontamination <br /> equipment to any area of f cility operation in an emergency. <br /> Violation #401 - Failed to tore hazardous waste in a container that is in good condition. <br /> Inside: <br /> -multiple double stacked ro s of plastic totes seen with free flowing oil on the ground (oil identified by Mr. <br /> Kandathil). <br /> Outside: <br /> -near the cyclone fence, by the hazardous waste storage area a 55 gallon drum with a hole in the bottom was seen. <br /> The contents appeared to a used oil and had drained onto the ground and into the dirt. <br /> -two damaged 55 gallon dr ms (green and red) appeared to have drained most of their contents, used oil, onto <br /> concrete and dirt. Near the red and green drums was a row of double stacked 55 gallon drums (blue), at least <br /> seven deep. Near the sixth set of drums was a dark oily stain on concrete. It could not be determined which drum it <br /> had come from. <br /> Hazardous waste shall be s ored in a container that are in good condition. Immediately transfer all hazardous <br /> wastes into containers that re in good condition or have hazardous wastes properly disposed of by a licensed <br /> hazardous waste hauler un er a proper manifest. <br /> Violation#403 - Failed to eep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> Inside: <br /> -one 55 gallon container wit an oily label "drained used oil filters" near the used oil filter crusher full of used oil open <br /> -four 5 gallon buckets of us d oil, oil absorbent, identified as non-RCRA hazardous waste <br /> -one 55 gallon blue drum st ring mixture of used oil, oily rags and equipment <br /> -one 55 gallon drum of wast aerosol cans, identified by Mr. Kandathil, unknown source <br /> -two thirty gallon drums and one five gallon bucket stored near the east wall of the building, identified as non-RCRA <br /> hazardous waste by Mr. Kar dathil <br /> Outside: <br /> -one 55 gallon drum labeled "used absorbents" with a generator name of"williams tank lines" containing used oil <br /> and other hazardous waster, identified as non-RCRA by Mr. Kandathil <br /> -one 30 gallon black drum o used oil in the "hazardous waste storage" area <br /> All hazardous waste contai ers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and insure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation #404 - Failed to i spect hazardous waste storage areas at least weekly. <br /> Weekly inspections of the c instruction materials, fixtures, and surrounding areas of the hazardous waste tanks <br /> were not performed. Immed ately begin to conduct weekly inspections of the construction materials, fixtures, and <br /> surrounding areas of the ha ardous waste tanks. <br /> Page 5 of 8 <br />
