The following is z n itemized list of aboveground petroleum storage act violations that
have not been addressed for Global Materials Management, LLC as of April 17,
2017.
Open violations from August 26, 2016 inspection
Violation #712 - Failed to test or inspect each container for integrity based on industry standards.
Integrity testing was not co ducted on the aboveground oil storage containers on site. Each aboveground container
shall be tested and inspec d for integrity on a regular schedule and whenever repairs are made. The qualifications
of personnel performing to is and inspections, frequency and type of testing and inspections that take into account
container size, configuratio , and design shall be determined in accordance with industry standards. Examples of
these integrity tests include but are not limited to: visual inspection, hydrostatic testing, radiographic testing,
ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records and
other records of inspection and tests must be maintained on site. Immediately conduct the necessary testing and
submit a copy of the test r ults to the EHD, or provide equivalence as allowed by CFR 112.7(a)(2).
Violation#715 - Failed to egularly test liquid level sensing devices to ensure proper operation.
Liquid level sensing device have not been tested. Procedures and frequency of testing for these devices were not
addressed in the Spill Prev ntion, Control, and Countermeasure (SPCC) plan. Liquid level sensing devices must be
installed in accordance wit CFR 112.8 and shall be regularly tested to ensure proper operation. Immediately
conduct all necessary testi g of liquid level sensing devices, or provide equivalence as allowed by CFR 112.7(a)(2).
Violation#717 - Failed to romptly correct visible discharges and/or remove accumulations of oil in diked
areas.
Several drums were obse d with holes in the sides, leaking oil onto the ground in the yard. Visible discharges
which result in a loss of oil om a container, including but not limited to seams, gaskets, piping, pumps, valves,
rivets, and bolts, must be p omptly corrected. Immediately fix leaks or move all oil to non-leaking containers, clean
all stained areas and conta inated soil, and manage according to the California Code of Regulations Title 22
hazardous waste regulation .
Violation #718 - Failed to rovide sufficient secondary containment capacity for mobile or portable
containers.
The oil storage drums were observed with insufficient secondary containment. No secondary containment was
observed for the 180+ drum of used oil located on the dirt in the yard. Portable oil storage containers must be
positioned or located to pre ent a discharge and shall be furnished with a secondary means of containment
sufficient to contain the cap city of the largest single container with sufficient freeboard to contain precipitation.
Immediately provide sufficie t secondary containment for this and all other portable containers at this facility, or
provide equivalence as allo ed by CFR 112.7(a)(2).
Violation #719 - Failed to I cate mobile or portable containers to prevent discharge.
The drums of used oil were of located to prevent a discharge. No secondary containment was observed for the
180+ drums ofused oil, whi h were placed on the dirt in the yard. Portable oil storage containers must be
positioned or located to pre ent a discharge and shall be furnished with a secondary means of containment
sufficient to contain the cap city of the largest single container with sufficient freeboard to contain precipitation.
Immediately relocate these nd all other portable containers at this facility to prevent a discharge.
Violation #726 - Failed to equately warn vehicles entering facility to protect piping and other transfer
operations.
No warnings were observed on site to warn vehicles of aboveground piping, containers, or other appurtenances. A
warning must be provided to all vehicles entering the facility to be sure that no vehicle will endanger aboveground
piping or other oil transfer o erations. Immediately provide a warning in accordance with CFR Part 112 for all
vehicles entering the facility, or provide equivalence as allowed by CFR 112.7(a)(2).
