Mr. Kandathil is waiting to train <br /> employees until he feels tt iey will "work out'. Personnel who handle hazardous waste shall be trained within six <br /> months of assignment anc take part in an annual review of the initial training received. At a minimum, the training <br /> program shall be designe to ensure that facility personnel are able to respond effectively to emergencies by <br /> familiarizing them with em rgency procedures, emergency equipment, and emergency systems, including where <br /> applicable: <br /> 1. procedures for usin , inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for c utomatic waste feed cut-off systems; <br /> 3. communications or, larm systems; <br /> 4. response to fires or xplosions; <br /> 5. response to ground ater contamination incidents; and <br /> 6. shutdown of operati ns. <br /> The owner or operator sh II maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job descriptic n for each position listed above; <br /> 3. written description o the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that docuME nt that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept f r at least three years from the date the employee last worked at the facility. Immediately <br /> provide employees with initial or refresher hazardous waste training. Submit a copy of the roster and the syllabus to <br /> the EHD. <br /> This is a Class II violation. <br /> 119 HSC 25160.2(b)(3) Failed o keep copies of consolidated manifesting receipts for three years. <br /> Per Mr. Kandathil the business has been operating at this location for"a couple, 3-4 months". At time of inspection <br /> copies of consolidated ma ifests were not available for review. <br /> A generator shall keep a copy of each consolidated manifest for at least three years. <br /> Within 30 days, provide thQ EHD with copies of all consolidated manifests from this location. Begin the practice of <br /> maintaining consolidated anifests for a minimum of three years. <br /> This is a Class II violation. <br /> Page 6 of 15 <br />
